What happened
The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.
The event was reported as an engine failure, however, what actually occurred is that the left engine was intentionally shut down for engine out training and upon completion of this phase of instruction, the crew could not re-start the engine. The aircraft returned to base (gyr) without incident and maintenance personnel checked out the aircraft. The aircraft is owned and operated by cae oxford aviation academy and is used for flight instruction, however, this particular flight was a 14 CFR part 61 training flight and not part of the 141 curriculum.
This inspector spoke to maintenance personnel about the issue on 08/29/2013, at the gyr facility. This inspector was told that after the aircraft returned to base, the mechanic had no problem starting the engine and that it checked out good. The engine had been shut down on purpose, during training and no failure has been discovered or recorded. A copy of the only document recording the discrepancy and the corrective action, a technical logbook (tlb) (form oaa tlb form 1) page, contains the following write up by the PIC:
"Could not restart l-engine after intensional shutdown."(SIC)
The corrective action documented on the tlb is as follows:
"Reset prop to high pitch, started engine normally. All systems check ok."
The PIC / CFI was not available for interview.
On 09/06/2013, this inspector visited the ffz facility and spoke to the chief flight instructor, jerod bybee, for cae oxford aviation academy. Mr. Bybee provided this inspector with a copy of the aviation academy's air safety report, form cgaph-S-F001 that was filed for this event. The detailed description of the event contained in the report reads as follows:
"The left engine would not restart, with numerous attempts, after an intentional shut down, while finishing assymetric lesson number one."(SIC)
There appears to have been no failure, no damage and no injuries. No further action planned.
As the FAA filed it
THE EVENT WAS REPORTED AS AN ENGINE FAILURE, HOWEVER, WHAT ACTUALLY OCCURRED IS THAT THE LEFT ENGINE WAS INTENTIONALLY SHUT DOWN FOR ENGINE OUT TRAINING AND UPON COMPLETION OF THIS PHASE OF INSTRUCTION, THE CREW COULD NOT RE-START THE ENGINE.
