What happened
The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.
On 4/21/2014 lear jet N72CK scraped runway 24R at Burke Lakefront Airport with the belly of the aircraft. This was caused by the crew forgetting to extend the landing gear and completing the before landing check list. After noticing a scraping sound during the landing roll out, the pilot flying (privacy data omitted) chose to advance the power and commence a touch and go.
The following statement was provided by the director of operations for kalitta charters:
During a training flight in preparation for a check ride privacy data omitted completed airwork in the vicinity of dexer intersection and practiced two approaches into yip, the vor-a with a circle to 23L and the GPS (RNAV) 23L. Both of these approaches ended in missed approaches. After the RNAV approach they received a clearance and continued on to Burke lake front airport (kbkl) for practice ILS approach to runway 24R. When turning onto the approach privacy data omitted reduced power on privacy data omitted right engine to idle in order to have him perform a single engine ILS to a full stop at kbkl. Upon reducing power, the gear warning horn sounded and privacy data omitted silenced it with the switch located on the gear panel. privacy data omitted Called for the checklist and they performed the single engine landing checklist to include increasing vref 10 knots and computing landing distance plus 20% and lowering the flaps to a maximum of 20 degrees (approach flaps). They determined there was enough runway to land and stop safely based on part 91 numbers and continued the approach. While on the approach they brought the aircraft down to dh whereprivacy data omitted informed privacy data omitted that he had the approach lights in sight and to continue tracking the ILS. At 820' privacy data omitted told privacy data omitted the runway is in sight and to continue landing. In the flare a loud noise sounding like a blown or frayed tire caused privacy data omitted to initiate a go around. They went to the north of the field over the lake and visually held while the city could check the runway condition. No tire was found. They decided to return to base and have the aircraft looked at by maintenance. Landing at yip was uneventful and they taxied in to kalitta charters' ramp without further incident
This statement was consistent with the findings of the investigation.
The investigation also revealed that that the crew was advised by ATC that there was a 7 inch piece of jagged metal found on the runway at bkl. This was consistent with the pictures taken during the investigation. The pictures show damage to the underbelly, flaps and antennas which was consistent with a gear up touch and go.
Kalitta charters immediately sent both crew members for retraining in a lear jet simulator. The training was conducted with an in house instructor with emphasis on checklist useage and CRM. The director of operations (Mr. Brad clark) was advised that the crew will be reexamined under 44709.

As the FAA filed it
ON 4/21/2014 LEAR JET N72CK SCRAPED RUNWAY 24R AT BURKE LAKEFRONT AIRPORT WITH THE BELLY OF THE AIRCRAFT. THIS WAS CAUSED BY THE CREW FORGETTING TO EXTEND THE LANDING GEAR AND COMPLETING THE BEFORE LANDING CHECK LIST. AFTER NOTICING A SCRAPING SOUND DURING THE LANDING ROLL OUT, THE PILOT FLYING (^PRIVACY DATA OMITTED^) CHOSE TO ADVANCE THE POWER AND COMMENCE A TOUCH AND GO.
THE FOLLOWING STATEMENT WAS PROVIDED BY THE DIRECTOR OF OPERATIONS FOR KALITTA CHARTERS:
DURING A TRAINING FLIGHT IN PREPARATION FOR A CHECK RIDE ^PRIVACY DATA OMITTED^ COMPLETED AIRWORK IN THE VICINITY OF DEXER INTERSECTION AND PRACTICED TWO APPROACHES INTO YIP, THE VOR-A WITH A CIRCLE TO 23L AND THE GPS (RNAV) 23L. BOTH OF THESE APPROACHES ENDED IN MISSED APPROACHES. AFTER THE RNAV APPROACH THEY RECEIVED A CLEARANCE AND CONTINUED ON TO BURKE LAKE FRONT AIRPORT (KBKL) FOR PRACTICE ILS APPROACH TO RUNWAY 24R. WHEN TURNING ONTO THE APPROACH ^PRIVACY DATA OMITTED^ REDUCED POWER ON ^PRIVACY DATA OMITTED^ RIGHT ENGINE TO IDLE IN ORDER TO HAVE HIM PERFORM A SINGLE ENGINE ILS TO A FULL STOP AT KBKL. UPON REDUCING POWER, THE GEAR WARNING HORN SOUNDED AND ^PRIVACY DATA OMITTED^ SILENCED IT WITH THE SWITCH LOCATED ON THE GEAR PANEL. ^PRIVACY DATA OMITTED^ CALLED FOR THE CHECKLIST AND THEY PERFORMED THE SINGLE ENGINE LANDING CHECKLIST TO INCLUDE INCREASING VREF 10 KNOTS AND COMPUTING LANDING DISTANCE PLUS 20% AND LOWERING THE FLAPS TO A MAXIMUM OF 20 DEGREES (APPROACH FLAPS). THEY DETERMINED THERE WAS ENOUGH RUNWAY TO LAND AND STOP SAFELY BASED ON PART 91 NUMBERS AND CONTINUED THE APPROACH. WHILE ON THE APPROACH THEY BROUGHT THE AIRCRAFT DOWN TO DH WHERE^PRIVACY DATA OMITTED^ INFORMED ^PRIVACY DATA OMITTED^ THAT HE HAD THE APPROACH LIGHTS IN SIGHT AND TO CONTINUE TRACKING THE ILS. AT 820' ^PRIVACY DATA OMITTED^ TOLD ^PRIVACY DATA OMITTED^ THE RUNWAY IS IN SIGHT AND TO CONTINUE LANDING. IN THE FLARE A LOUD NOISE SOUNDING LIKE A BLOWN OR FRAYED TIRE CAUSED ^PRIVACY DATA OMITTED^ TO INITIATE A GO AROUND.
