From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

A review of information at hand provides:

(1) That jump appeared normal until the deployment sequence of main canopy.

(2) That suspension lines were twisted when main canopy deployed.

(3) That before twisted line problem could be resolved; reserve parachute system attempted to deploy and became entangled in main parachute lines.

(4) That parachute system was then uncontrollable and was not arresting the descent sufficiently on ground impact.

Parachute system:

Harness/container - mirage systems G3.

Main canopy - performance designs spectre.

Reserve canopy - performance designs pd reserve.

Aad - airtec cypres
