From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Aircraft took off on runway 15 with PIC and three passengers at 1815 local time. PIC stated that once airborne the aircraft would not climb. The PIC elected to cut the throttle to idle and attempted an emergency landing on airport property. The aircraft hit tail first and slid approximately three hundred feet before coming to rest. The aircraft came to rest on the threshold of runway 33 and came in contact with the precision approach path indicator (PAPI) array. Aircraft sustained substantial damage and one passenger had minor injuries.
