From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

A private pilot was receiving a check-out in a PA-28-181 from a flying club CFI. After completing three take-offs and landings, the pilot receiving instruction stated the instructor announced that they were going to perform a half power soft field take-off to simulate a high altitude soft field take-off. The pilot receiving instruction stated that while taxing to the runway, it was agreed that the instructor would control the throttle and the pilot receiving instruction would handle the flight controls. During the attempted take-off, directional control of the aircraft was lost. The aircraft drifted to the right of runway 36 over the grass, stalled, and struck the ground. The aircraft became airborne again only to stall and impact the runway surface, veering across the runway at taxiway bravo to the opposite side of the runway where it came to a stop.
