From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Pilot privacy data omitted selected gear up on take off from knsa, saw three green indicator lights went out, no red in-transit lights, and decreasing airspeed. He cycled the gear several times, but the unsafe condition remained. On approach into Burbank/Bob Hope Airport, privacy data omitted performed the emergency gear down procedures. When the circuit breakers were cycled he noticed the two main gear green lights flickered, but no nose gear light indication. Burbank tower confirmed all gear was down, after the pilot made a fly-by. Upon landing the nose gear collapsed. The aircraft sustained propeller and undercarriage damage.
