From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Jumper name paul m hayes.

Jumper experience approximately 750 jumps. Recent aff rating. Reserve side instructor proficiency check 4/18/15.

Jumper has no known club affiliations.

Parachute is a katana 107 manufactured by performance designs, owned and packed by skydiver, reserve not used/involved.

Jumper exited aircraft normally approximately 12,500 feet, executed several manuvers in freefall, deployed main parachute normally. On approach to landing zone jumper should have executed 270 degree turn before flare but executed hard high speed left turn for unknown reasons. Video evidence suggest inadvertent actuation of front left riser instead of standard toggle turn. The high speed turn resulted in the jumper contacting the ground at high rotational velocity, causing substantial injuries. The jumper died approximately one day later.
