From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

CFI was conducting pinnacle approaches at high altitude. CFI initiated shallow approach to pinnacle and slowed his forward airspeed to minimum as helicopter was about to touch down. CFI realized power was insufficient to cushion touch down and helicopter fell short of planned area, hit the ground and slightly bounced with a little forward momentum. Acft then tilted to the left and rolled over on left side and slid about 3 feet. CFI pulled mixed idle cut off and the engine stopped. CFI and student got out of aircraft without injury. CFI then notified mauna loa management.

ACFT THEN TILTED TO THE LEFT AND ROLLED OVER ON LEFT SIDE AND SLID ABOUT 3 FEET. CFI PULLED MIXED IDLE CUT OFF AND THE ENGINE STOPPED. CFI AND STUDENT GOT OUT OF AIRCRAFT WITHOUT INJURY.
