From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

The main parachute failed to completely open. A witness stated that a line over the chute prevented it from properly opening. When the parachute jumper tried to use the cut away device, the right side riser cut away properly and the left riser remained attached to the pack. The deployed but spinning chute material twisted the left riser to the point that it prevented the remaining cutaway cable from operating at all. The paracute jumper then deployed his reserve chute just above the tree top level but too late to deploy.
