From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

On Friday/20NOV15 at approximately 15:30L pilot stated he was solo landing a PA-11 cub special, N4681M, at kbtv on rwy 01. Weather was cavu, winds were 300/10. Fuel tank was full (16GALS), with approx 10LBS baggage behind aft seat. He weighs 185LBS.

There was light turbulence on final, so approach was flown at 65-70MPH indicated. Touchdown was smooth on centerline about 150' past the threshold at approx 60MPH, with crosswind controls (left aileron, right rudder), on the main gear with the tail raised.

As he applied braking the tail raised up more quickly than he was expecting, so he eased off the brakes and added back stick. As the tail lowered he began reapplying brakes, but as the aircraft slowed to taxi speed (still with tail raised), the tail raising higher with full aft stick. The aircraft forward motion was almost stopped as the extreme tail-high attitude put the aircraft CG forward of the main gear, the idling prop struck the runway, and the aircraft gently came to rest nose-down on the prop spinner and two main gear. The aircraft was on centerline approx 800' past the threshold (50' past taxiway c intersection).

He radioed btv, tower I had closed RWY01, and shut off the fuel, mags, radio, and iff.

By lifting up on the prop he was able to smoothly return the aircraft to a normal three-point attitude.

As the FAA filed it
