From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Aircraft was being flown as a test flight after completion of a annual inspection. Slightly before rotation speed (aprox 80KTS) the nose gear collapsed casing minor damage to the underbelly and prop strikes on both engines. A departing piece of prop also damaged the leading edge of the right wing half way down the wing.

Aircraft annual inspection was not endorsed prior to the test flight action. The aircraft had been issued a new n# several years ago and the airworthiness cert was never updated.
