From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

The CFI was demonstrating an auto rotation while conducting a flight review. The maneuver was initiated at approximately 1200 feet above ground level (AGL). During the maneuver, the CFI was not satisfied with the engine and rotor indications and initiated a power recovery by increasing the throttle. As the CFI increased the throttle, the CFI noted that the engine was not responding appropriately. The CFI flared at approximately 50 feet AGL and continued to apply throttle and collective. At approximately 10-15 feet AGL, the helicopter lost lift and impacted the ground.
