From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

On July 25th, 2016, approximately 40 minutes into a flight from mdw to hpn, ATC contacted the pilot notifying him that a wheel came off of an aircraft during take-off and was possibly from his aircraft (N721G). Ground personnel at mdw recovered the wheel, and after relaying the information to the d.o.m., it was determined to have come from the #1 position of the main landing gear of N721G. After the determination was made that the wheel came from N721G, the pilot reduced speed and declared an emergency to land at bdl. The pilot received visual verification from ground personnel at bdl that the wheel was missing from the #1 position and landed at bdl without incident.
