From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

During training flight with CFI and student. CFI instructed student to pick an off airport landing site for emergency landing. Student selected road in clear cut logging area and initiated approach. During later stage of approach CFI stated "it did not feel right'" and assumed control of aircraft. The aircraft began to yaw right, CFI applied forward cyclic. Aircraft spun 1.5-2.0 turns before CFI rolled throttle to detent, and pulled collective just before aircraft settled to ground. Aircraft rolled to left side on uneven terrain where rotor impacted ground. Aircraft fuel and switches where selected off and both occupants exited aircraft with no injuries. CFI has since voluntarily resigned from flying.
