What happened
The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.
On September 12, 2017 at approximately 1910MST, N886MA a beechcraft/raytheon BE-1900D aircraft operating under FAR 135 by maverick airlines, inc. experienced a collapsed nose wheel landing gear upon landing at Grand Canyon national park airport (kgcn).
The preliminary investigation has revealed that the aircraft departed kgcn en-route to henderson executive airport (khnd) when the aircraft began to experience multiple electrical failure indications including failure of the left engine generator. The flight requested and initiated a return to land at kgcn. Prior to landing, it was determined by the flight crew that the wing flaps were inoperative, however the landing gear was lowered and indicated ?down and locked? By three illuminated green lights. The control tower also observed the landing gear was extended. A landing was made on runway 03, but upon roll-out the nose wheel landing gear collapsed causing substantial damage to the forward fuselage skin, nose wheel landing gear doors and both engine propeller blades.
The pilot and co-pilot initiated the evacuation of the aircraft. Both pilots and 18 passengers were not injured. One passenger received minor injuries and was treated at the scene by responding airport ARFF personnel.
Initial investigation is ongoing. Investigators from the FAA and NTSB will continue the investigation.
The aircraft is owned by mustang leasing, LLC.
As of 10/11/2017, the operator is still waiting for the engines and props to get back from o/haul, so systems/failure forensics are on hold at this time.
