What happened
The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.
On September 22, 2017, N618WF initiated a departure from kmez (mena mountain municipal airport, AR) for kphf (Newport ? Williamsburg international airport, VA). After engine start up, the ?gen 1 fail? Cyan message displayed, the flight crew determined that the item was listed in the BD-700-1A/10 mmel under item number 24-21-01 and departed kmez at 20:06Z.
Enroute to kphf, between 20:12Z and 22:06Z a total of 5 fight and instrument display units (du pn: 7014300-901) screens flashed off and on then blacked out at different times throughout the flight. The du black out sequence was (6, 3, 5, 4, and 1 without flashing).
The following additional cas messages were observed during the flight:
L bleed fault cyan message displayed.
Rud authority low amber cas message displayed.
Rudder limiter fail amber cas message displayed.
Left windshield heat amber cas message displayed.
On approach to (kphf) at 8,000 feet the flight crew retarded the engine throttles to idle, the left engine continued to decelerate and the ?left engine flameout? Amber cas message displayed. The flight crew accomplished the remaining steps to shut down the left engine and landed uneventfully at 22:14Z.
The company director of maintenance, a maintenance technician; a Bombardier technical representative and rolls royce maintenance technicians responded to the aircraft for the investigation. The NTSB, airframe, engine and component manufacturer engineering held conference calls between September 25 and October 11, 2017 to support and direct the investigation.
During the investigation the following items were identified:
The left engine variable stator vane actuator (vsva) p/n 2606 MK3 had a dual lane lvdt fault causing the electronic engine control (eec) logic to shut off fuel to the engine.
A leak in the avionics cooling fan suction ?u? Duct at the fan motor manifold caused four of the six Honeywell DU-870 primary instrument and navigation display units to fail due to overheat.
A dcpc contactor-K3(pn MFR58657-0027) failed closed on aircraft power up at kmez causing the ?gen 1 fail? Cyan message, ?l bleed fault? Cyan message, ?rud authority low? Amber cas message; and rudder limiter fail amber cas to be displayed prior to departure or during the flight to kphf.
The investigation continues into the component analysis phase. This includes the leach K3 relay with utas, the four DU-870 display units with Honeywell and the two vsvas with rolls royce. These activities will be monitored by Bombardier and the NTSB.
As the FAA filed it
ON SEPTEMBER 22, 2017, N618WF INITIATED A DEPARTURE FROM KMEZ (MENA MOUNTAIN MUNICIPAL AIRPORT, AR) FOR KPHF (NEWPORT ?
