What happened
The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.
The pilot stated that on approach he was going too fast and attempted to stay in ground effect to reduce speed. At that time, the co-pilot seat back fell forward. The pilot stated, that while holding the seat back up, "my body must have pushed the yoke ahead and hit the runway very hard!" This impact created multiple wrinkles in the skin and structure of the aircraft, including under the windscreen and around the fuselage behind the crew doors.
