From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

A student pilot during check-ride for a private rating with designated pilot examiner (DPE) onboard, crashed on take-off from rwy 17L at kapa airport. For the initial takeoff, the DPE asked the applicant to perform a simulated soft field take-off maneuver. Airfield video shows the aircraft lifted off the runway and climbed about 10-15? Above the runway, leveled off then sank back, close to the runway.

The aircraft slowly began to gain altitude, the aircraft increased in a nose up attitude, which induced an aerodynamic stall. The aircraft veers left of the runway (left wing low), prior to impact, the aircraft rolls right impacting the ground in a right angle and nose low. Severe damage to the nose gear, firewall, and right landing gear. The fuselage structure aft of the cockpit is twisted and buckled.
