From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

Narrative of events:

Drove to lafayette airport after dark with my wife. Got the rental book from privacy data omitted and went out and executed pre-flight inspection of N6208T. Everything normal and boarded aircraft. Requested VFR flight following and taxied out to runway 4R. After normal run up and all checks complete taxied onto runway 4R, pulled back on yoke to reduce minor shimmying and slowly brought yoke back to regular position for departure exiting to the north. Routine flight to Marksville municipal and made normal landing. Taxied to main building tie down area. Shutdown engine and departed aircraft for bio break. Another inspection of the aircraft, boarded for return to lafayette regional. Normal startup and announced my intentions while taxing to runway 4. Taxied to hold line on ru way 4 for a run up. All checks complete and announced my intentions on the common frequency· turned onto runway, pulled the yoke back to eliminate shimmy and started to add power, checked engine performance, added more power. Nose seemed to drop a bit, added more power. Checked engine performance again, hand on throttle and yoke, then nose dropped much lower then made ground contact. Pulled power, aircraft skidded down runway swerving to the left and stopped in the grass. Turned off key, master switch, removed myself and wife from aircraft. Called privacy data omitted and airport manager privacy data omitted.
