What happened
The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.
The pilot of this light sport aircraft was attempting to perform a soft field takeoff with a left crosswind of approximately 8KTS (maximum crosswind limitation of 10KTS for this aircraft) as a part of a practical test for a sport pilot certificate. The pilot failed to make the immediate control inputs (left rudder and/or left wing down) to compensate for the drift to the right as the main landing gear came off the ground on takeoff. The slower forward speed of the soft field takeoff profile resulted in insufficient lift - the aircraft lost the small amount of altitude acquired and the nose & right main landing gear contacted the runway (in a drift to the right) with the right wingtip contacting the ground immediately thereafter. The aircraft came to a stop on the runway.
As the FAA filed it
THE PILOT OF THIS LIGHT SPORT AIRCRAFT WAS ATTEMPTING TO PERFORM A SOFT FIELD TAKEOFF WITH A LEFT CROSSWIND OF APPROXIMATELY 8KTS (MAXIMUM CROSSWIND LIMITATION OF 10KTS FOR THIS AIRCRAFT) AS A PART OF A PRACTICAL TEST FOR A SPORT PILOT CERTIFICATE.
