From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

ATC reported to FSDO: Las Vegas, NV (las): OKC436, BE40, private jets 436, aus - rno, diverted to las after declaring emergency due to pressurization issue, landed without further incident. 08/14/2021 1750Z.

The pilot reported to the iic: enroute from kaus to krno at 40,000 feet, the pilot in command (PIC) observed a 200 feet per minute climb in cabin altitude. The PIC stated they increased the cabin controller rate knob in an attempt to level off the cabin pressure, with no effect. The PIC stated they instructed the second-in-command (SIC) to request a descent to 20,000 feet from ATC. The PIC stated they selected both high on the environmental control system (ecs) selector, and the cabin began climbing at 500 feet and increasing. The PIC stated that clearance to 20,000 feet was given by ATC and the PIC stated they made the decision for the crew to don oxygen masks, declare an emergency and the crew conducted emergency descent procedures to 10,000 feet, while the cabin depressurization rate increased uncontrollably to 2500-3000. The cabin altitude reached 14-15,000 feet during the descent and cabin oxygen masks deployed. The PIC stated they diverted to Las Vegas, and upon reaching 10,000 feet, they canceled the emergency and conducted a normal landing at Las Vegas (klas) airport without further incident.
