From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

On 06/02/2022 at approximately 3:00PM PDT, Bell HH-1H N91JK, departed Reno-stead airport to conduct familiarization/exercise training for the washoe county sheriff?s office. At approximately 4:00PM PDT, pilot and copilot were conducting off field takeoffs and landings. On the second takeoff the pilot's left steel toed boot got stuck under the left anti-torque pedal, and was caught on the front of the foot rest panel at the heel of the boot. This resulted in uncontrollable left rotation, and when the aircraft was settling on the ground it continued left rotation for approximately 2 turns collapsing the skids and coming to rest on its belly. Aircraft sustained substantial damage to the fuselage bulkheads, and tail boom. No post-accident fire.
