From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Plane departed from kalskag enroute to Aniak Alaska. Pilot noticed the landing gear warning light was illuminated, cycled the gear a couple times however the light remained on. Pilot pulled the bulb to distinguish the light. Pilot believed it would distract me from other parts of his flight and didn’t want to become fixated on the “light problem”. Pilot accidentally dropped the bulb which distracted him for the rest of the flight. Pilot admitted to being distracted by traffic inbound to Aniak from the opposite direction. Pilot prepared to land, however forgot to lower the gear and never referenced the checklist. In the landing flare pilot realized the “three green lights weren’t illuminated” (gear not down and locked) and initiated a go-around. Upon starting the go-around pilot heard a grinding sound initially believed at the aft portion of the aircraft. Pilot stayed in the traffic pattern and landed uneventfully on the second attempt. Upon landing pilot discovered a bent prop.

As the FAA filed it
