From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

Pai privacy data omitted and ASI privacy data omitted arrive at stick and rudder privacy data omittedmaintenance to investigate incident at 0900. Aircraft was on wheels in the hangar. Mx staff testified that they had c/w gear swings and the gear operated normally. They also stated when power was initially applied to the aircraft the gear warning horn was sounding and during swings it operated properly. Mx staff stated the gear horn volume was quiet, and they did not believe it would be heard with bose active noise cancelling headsets on.

Investigation of the aircraft revealed prop damage consistent with an engine running ground strike. Scraping damage was noted on aircraft lower fuselage primarily on the frame where the wing struts attach to the fuselage and on the frame at the aft end of the cabin. It is unknown if scraping damage affected the aircraft structure at this time. Scraping damage was also noted on tail tie down ring and the ring was bent aft approximately 30° with radial skin cracks extending from ring to surrounding rivets. No damage to aft bulkhead was noted at the time but further investigation will be required by maintenance. See “N711PD damage photos” folder.

ASI privacy data omitted interview privacy data omitted by phone starting at 1029. privacy data omitted Stated the aircraft is based in falmouth ma, weather conditions were clear and dry and that the flight was for pleasure. His 12 year old granddaughter was a passenger on this flight. He has been a pilot for 34 years and owned this aircraft for 24 years. He stated that he successfully performed a touch and go on runway 24 and departed left cross to make a second landing. He only retracted the flaps to 10° while in the pattern. Per his description of events he was calling out his landing checklist and looked out his window to see the flaps were already deployed. He believes that this deviation from the normal landing routine caused him to miss steps in the checklist. He says he knows he called out “gear down” but due to the upset in the usual landing sequence he never actually reached for the handle and deployed the gear.privacy data omittedStated he could not hear the gear warning horn in the cabin, not did the warning transmit into the headphones. The aircraft landed, gear up on runway 24 at approximately 1015.

privacy data omitted Was very forthcoming, amiable and cooperative during the interview.

ASI privacy data omitted determined it was not necessary to interview the granddaughter at this time.
