From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

On February 23, 2023 at approximately 11:48 eastern time, the pilot rotated at 45-50 mph and never went above 40 mph while attempting to takeoff on runway 17 at Twelve Oaks private airfield (II87) in a Piper PA-28 privacy data omitted. In her attempt to climb, the aircraft settled back to the ground and the pilot rotated a second time. The airspeed of the aircraft would not increase due to the slow speed of approximately 40 mph. Fearing a stall, the pilot released back pressure on the yoke and pushed the aircraft nose down to attempt to land. The aircraft impacted the ground and struck a bush on the right wing, spinning the aircraft 180 degrees. The aircraft then slid down a hill and came to a stop approximately 50 yards off the end of the runway. The aircraft sustained substantial damage. The early rotation was below stall speed and created a loss of control situation.

As the FAA filed it

ON FEBRUARY 23, 2023 AT APPROXIMATELY 11:48 EASTERN TIME, THE PILOT ROTATED AT 45-50 MPH AND NEVER WENT ABOVE 40 MPH WHILE ATTEMPTING TO TAKEOFF ON RUNWAY 17 AT TWELVE OAKS PRIVATE AIRFIELD (II87) IN A PIPER PA-28 ^PRIVACY DATA OMITTED^.
