From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Pilot reported: flew the pattern one time to test radios. Landing gear was not selected up during any phase of the flight. After a normal landing while the airplane was slowing down, pilot decided to do another flight over the pattern and he selected gear handle instead of the flap selector. After the gear horn sounded, he quickly selected gear down. He reached over the flap selector and selected take off flap configuration, applied power for takeoff when the nose gear collapsed striking the prop on the ground coming to a full stop.
