From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

141 Flight piloted by a CFI (PIC) was providing high altitude mountain training instruction to commercial/cfi student . The student pilot attempted to execute a pinnacle landing at approximately 6600 feet MSL in mountainous terrain (edge/ridge of a small volcano near hualalai). Density altitude at point of impact was estimated at 7600 ft and temperature estimated to be between 16-24 degrees celsius. During final moments of approach the rotor RPM drooped with insufficient power to arrest the descent. The instructor came on controls and initiated a go around but was too low. Aircraft collided with the ground, bounced, broke off left gear and then rolled over on left side approximately 100 feet short of the intended landing area. Student after roll over secured engine and then both pilots evacuated aircraft uninjured.

As the FAA filed it
