From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Pilot reported that upon brake release and applying full power for take-off, her seat slid all the way back to the "full back" position. The pilot was subsequently removed from the aircraft controls as the aircraft began it's take-off roll. With no one controlling the aircraft, the pilot was able to use her toe to reach the throttle and pull herself up to regain throttle control and get herself behind the yoke in an attempt to regain control of the aircraft. The aircraft crossed the taxiway at the end of the runway and came to a hard stop in a ravine at the end of the runway.
