From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Shortly after takeoff the crew received a RH break fault message. The crew did the required qrh procedures after climbing through ten thousand feet. After that, the flight was uneventful until the crew tried to put the gear down on approach at dro. When the crew tried to put the gear down, they received a landing gear disagree eicas message. At that point the crew did the discontinue procedure and entered a holding pattern. The crew ran the qrh procedures but were still getting the message that the right main landing gear was not down and locked. By the time the crew was done with qrh procedures, they didn’t have the fuel to go anywhere else. The crew informed the fas to prepare for an emergency landing and declared an emergency. The crew informed ATC that they were ready for the approach. On final, ops observed that the gear was down. Aircraft continued the approach and landed safely. Aircraft taxied off the runway and the crew told the flight attendants that the aircraft was safe. The aircraft was inspected by ops and tugged to the gate. It was determined that the MLG tire failed during take-off at phx.
