From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Shortly after initiating a departure roll on rwy 19 at inf the pilot lost control of the aircraft, veered left and exited the runway at full power. He then traveled down a slight embankment, across the grass toward the taxiway at approximately an 80-degree angle to the taxiway, becoming airborne due to the slope of the turf next to the taxiway. The aircraft traveled airborne across the taxiway and contacted a concrete berm approximately 4 feet from the base in a nose up attitude, as evidenced by the witness marks. The aircraft bounced, became airborne again and traveled up the berm and came to a stop when contacting the airport perimeter fence coming to rest on a roadway guardrail.

The pilot received serious injuries to his legs, back and face and was airlifted to the nearest trauma facility. The passenger received minor injuries and was able to walk to the ambulance enroute to the local hospital.

As the FAA filed it
