From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

During the takeoff roll in kpsp, above 80 knots and below V1, the pilot monitoring (pm) observed a caution light illuminated with no associated instrument indication. The captain elected to continue the takeoff. After takeoff, the issue was identified as the no. 1 engine integrated drive generator (idg) light. The auxiliary power unit (APU) assumed the second electrical source, and the issue was communicated to maintenance control center (mcc). After leveling off, the APU low oil pressure light illuminated, and the APU shut down automatically as designed. The aircraft was then downgraded to a single electrical source, and the crew declared an emergency and diverted klas.

Investigation findings / observations:

• During maintenance inspection of the idg, magnetic particles were discovered.

Corrective action (CA) / preventative action:

• Aircraft was removed from service

• APU lube module was replaced. APU tested serviceable.

• Idg filter inspection carried out with magnetic particles found. Air cooler lines were flushed with no faults were found. Idg was replaced.

As the FAA filed it
