Probable cause

The CFI failed to maintain a safe sink rate resulting in an undershoot of the runway.

Sequence of events

Analysis

Returning from a practice session, the CFI declared a simulated engine out procedure. As the airplane entered a close left base, severe sink rate was encountered. The CFI was unable to recover from the sink rate and the airplane hit short of the runway. The airplane then slid onto the runway turning to the right and losing the right wing and nose gear.
