INVESTIGATION OF INCIDENT SHOWS THE ROD END HAD BROKEN AT THE BOLT HOLE. THE ROD END SHOWED SIGNS OF COLD WORKING. SUBMITTER RECOMMENDS A NDI TEST BE PERFORMED ON THIS PART OR A TIME LIFE BE PLACED ON THE PART.
