What was found
(AUS) CAPTAINS INSTRUMENT PANEL DISLODGED AND MOVED REARWARDS DURING TAKEOFF. INVESTIGATION FOUND THAT THE PANEL HAD NOT BEEN CORRECTLY SECURED FOLLOWING MAINTENANCE. PERSONNEL/MAINTENANCE ERROR.
