DURING INSTALLATION, MAINTENANCE DETECTED THE HEX HEAD SCREW COTTER PIN HOLE WAS HIGHER/CLOSER TO THE TOP/HEAD OF THE BOLT WHICH PREVENTED THE COTTER PIN FROM GOING THROUGH.
