From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.
What happened
The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.
Narrative: Mr. Wilbert j. tovrea, certificate #482487931, was the pilot-in-command giving flight instruction to susan e. hatherill, a student pilot. He allowed Ms. Hatherill to execute recovery from a tail rotor malfunction. Mr. Tovrea initiated the loss of tail rotor thrust condition by centering the anti-torque pedals. The aircraft yawed right. The student used an improper recovery by adding excess collective pitch during the engine out. The aircraft touched down on the forward part of the skids, rocked backwards, then the main rotor contacted the tail boom and tail rotor drive shaft leaving dents in both. The instructor was unable to take the controls in time to avoid the incident. This incident is closed with this report.
