From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Narrative: during approach to land on runway 19L at Richard l. Jones, jr., airport (rvs), Tulsa, Oklahoma, the pilot lowered the landing gear and verified three green "gear down" lights and checked the visual indicator which also showed the gear was down. The pilot stated that he made a normal landing and during roll out the nose gear collapsed. An air traffic controller on duty at rvs tower reported observing N5481D on final approach and that the landing gear appeared to be down and in place. A mechanic who inspected the aircraft after the incident found damage to the nose gear actuating rod that may have occurred from a previous hard landing.
