From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Narrative: on January 22, 1996, a smyth Sidewinder (experimental) N23ED operated by Mr. Arthur r. lewis landed at Chino Airport in CA. On landing, the nose gear collapsed. Minor damage was done to aircraft with no injuries to pilot. No landing gear down and lock indication was observed in the cockpit. Gear was verified down and locked visually by the pilot through an inspection window inside of aircraft. Another visual inspection was verified by air traffic personnel at Chino tower. Troubleshooting was conducted on the landing gear system. System analysis revealed the electro-hydraulic micro switches that control operation of extension and retraction of nose gear failed to operate and lock gear in proper position.
