From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Narrative: on April 6, 1996, at approx. 4 p.m., Cessna N4576Q experienced a right landing gear retraction after landing at the Hilo airport. The aircraft sustained minor damage to the prop, landing gear door and lower gear strut, wing tip and aileron. The incident was caused by the failure the cast aluminum fitting on the end of the push/pull retract tube. This fitting held a bearing in place thru which the landing gear was either push for retraction or pulled for extension. Examination of the fracture surface indicates a preexisting crack. This part, if original, had over 18,000 hours total time and many years in the Hawaii corrosive environment. Examination also revealed that the bearing had been restaked with a center-punch that probably caused a stress riser at the edge of the bearing hole. The factory stakes were done with a tool that would not result in stress risers. No log entry could be found that would trace this work to a person. This bearing was not designed to be replaced in the field. Cessna had no history on failures but did state they sold a lot of these assemblies. Closed. As the FAA filed it

CLOSED.
