From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Narrative: on takeoff the tread from number two main tire came off and was ingested into number one engine. Aircraft was below V1 so the takeoff was aborted. Equipment was called. There was no post-incident fire, no evacuation, and no injuries. Maintenance replaced number one engine due to FOD damage (both first and second stage fan blades were damaged), inboard aft flap (damage from tread), wing closer panel just above number two tire (damage from tread) and number two tire. After all items were replaced aircraft was returned to service. Dala cmo was contacted. This is a final report.
