What happened
The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.
Narrative: us airways flight 1446 departed bos enroute to phl on 4/7/97. The crew noted in the aircraft's log that "left lg downlock switch" fault indicated on takeoff from bos. This fault required no action other than the log book notation. On landing at phl both lmlg tires shredded causing damage to the left wing inboard flap shroud. On site investigation revealed that the lmlg downlock switch was broken at its' attach bracket to the gear. Evidence suggest that one tire may have been lost on take-off from bos. This would explain the lmlg down lock switch fault. That the crew received in the cockpit, as tire debris struck the lmlg. The second tire would have been lost upon landing at phl with the flaps fully extended. This would explain the damage noted to the flap shroud but none to the flap. Bos ATCT was contacted for information of any debris found on the runway that flight 1446 departed from. None was reported. The passengers were removed from the aircraft via a mobile airstair and bussed to the terminal. The aircraft was towed to a maintenance location at phl. Maintenance crews in phl removed and replaced both lmlg wheel assemblies. Both failed tires had been previously installed on 02/15/97 at ric and had 320 cycles logged since that installation. Phl maintenance then inspected the engines' inlet & exhaust and the entire airframe for damage. Reported damage to us airways. Pit maintenance, and obtained a ferry permit for the aircraft with the landing gear to remain down. The downlock switch was safety wired to the lmlg for the ferry flight to pit where all required correct actions would be accomplished. This incident is considered closed.
