From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Mr. Bastin was giving instruction to mr wright towards a complex endorsement. After several touch and goes, on takeoff, the nose gear collapsed causing the prop to strike the runway and stopping the engine. Both pilots had no recollection of raising the gear handle while on the ground. However, the gear handle was found in the up position. The landing gear system was inspected by a mechanic and no malfunction was found. Recommend both pilots receive counceling by the safety program manager. Safety recommendation of the landing gear system may be forthcoming.
