From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Operations inspector clyde b. o'neill interviewed jeffrey fletcher (PIC) and david junker (private pilot) on 12/28/98, at the Braithwaite airport (LA75). Inspector o'neill determined that a probable cause of the uncontrolled roll of the aircraft was due to a type of wave phenomena created by the prevailing wind being deflected over the Mississippi river levee. After the PIC initiated recovery from the stall/spin, the private pilot made opposing control inputs that probably contributed to the delay in recovery and resultant ground contact. Inspector o'neill determined that the PIC was knowledgeable of stall/spin recovery procedures and counseled him on related matters.
