From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) On October 10, 2001, bruce a. greig privacy data omitted sustained fatal injuries from a skydiving incident at Skydive Chicago, Ottawa, IL. A witness on the ground observed that Mr. Greig's parachute inflated at a normal opening altitude and immediately began spinning to the left, causing twisting of the canopy line's, thus rendering the main chute ineffective and the parachutists fell to the ground. The emergency chute was deployed at an insufficient altitude to achieve normal reserve inflation. Name of jumper: bruce a. greig, trained by Skydive Chicago's advance freefall program. Mr. Greig had 78 jumps. Club affiliation: Skydive Chicago, 3215 e. 1969th rd., Ottawa, IL 61350. Main chute--performance design, model--stiletto 135, serial no. ST135-010834. Date of manufacture: April 2001, and packed by Mr. Greig 10-13-01. The main canopy and line's appeared to be in airworthy condition and properly assembled to the risers. The right steering control was not pulled or released by the parachutists, a pull test of the right steering control was conducted using a pound's (PSI) pull tester and the right steering control released itself using three pounds of extraction force (within limits). See additional sheets.
