From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) On 11-04-2002, BE-76, N110SU, sustained damage due to collapse of the nose gear during takeoff roll. The aircraft was repaired and returned to service by aircraft maintenance specialists of Ocala, Florida. On 04-28-2003, during the first takeoff after the aircraft was returned to service, the nose gear again collapsed during takeoff roll, just prior to rotation speed. The pilot instantly applied nose up pitch to prevent the aircraft from striking the runway. The aircraft became airborne with minor damage limited to the tips of the left propeller. The pilot continued the takeoff, and after reaching a safe altitude, manually extended the landing gear, and returned for landing without further incident. Investigation of this incident by FAA airworthiness inspectors revealed that personnel of aircraft maintenance specialists had performed inadequate repairs, to specifically include failure to correct improper rigging of the nose gear assembly of N110SU, following the first nose gear collapse incident on 11-04-2002.

As the FAA filed it
