From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) On climb out from Phoenix Sky Harbor International Airport to minneapolis, Minnesota at flight level 190 there was a loud bang and an airframe shudder. The pressurization was checked and the outflow valve was found to be full closed while the cabin altitude was climbing. The crew initiated an emergency descent, declared an emergency and returned to Phoenix, without further incident. Maintenance personnel found a blown out duct in the right air conditioning bay. The check valve that prevents reverse airflow from the mix manifold also failed. The flapper disk was torn off and found in the ducting. That is why the airplane pressurization could not be maintained. Maintenance personnel removed and replaced the duct assembly, part #213N403 and the check valve, Garrett part #3202142-1.
