From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) On 11/30/03, during flight from Bethel to aniak, Alaska, the pilot noticed a smell in the cockpit and a rise in oil temperature. Pilot pulled the manifold pressure back to 23 inches and within a few minutes the prop went to high RPM. Pilot made a 180 degree turn back to Bethel. The engine continued to lose power and the pilot decided to land when the oil pressure dropped to zero. The aircraft was forced to land on a lake near akiachak, Alaska. (-23 amendment) privacy data omitted : inspection of this engine revealed #1 cylinder deteriorated due to fuel starvation from a clogged fuel injector leaning out the fuel mixture to this cylinder. Aluminum particles contaminated the oil system. The oil system feeds from the rear of the crankshaft to the front. Progressive oil pressure loss in the forward part of the crankshaft precipitated the oil starvation of #4 rod bearing which overheated and failed. The engine was shut down prior to complete separation of rod and crankshaft.

As the FAA filed it

(-23 AMENDMENT) ^PRIVACY DATA OMITTED^ : INSPECTION OF THIS ENGINE REVEALED #1 CYLINDER DETERIORATED DUE TO FUEL STARVATION FROM A CLOGGED FUEL INJECTOR LEANING OUT THE FUEL MIXTURE TO THIS CYLINDER.
