(-23) Skydive Hawaii fatal parachute jump, February 6, 2005, final report the parachutist exited the aircraft normally at approximately 11,000 AGL. Main parachute deployment was initiated low and resulted in a malfunction. The automatic activation device (aad) was on during the jump however the main parachute, although malfunctioned, slowed the parachutist enough that the aad did not activate and cut the reserve closing loop at the normal preset altitude. In addition, cutaway of the main parachute was accomplished too low in order for the parachutist to attain enough speed in time to activate the aad. The reserve was manually deployed however the parachutist was too low for the deployment sequence to complete. One reserve locking stow remained intact post impact. The parachutist was a 24 year old male, uspa authorized and current, with 171 jumps.
