From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Aircraft encountered a flock of birds on rotation. #1 engine flamed out, #2 lost RPM as the landing gear was retracted. Flight crew was unable to maintain altitude and airspeed. Aircraft contacted the ground just north of runway 25, sliding approximately 3000 feet through a fence, over a ditch and across a road, coming to rest in a cornfield. Both engines ingested numerous birds and debris, fodding out the #1 engine. The empennage separated from the fuselage approximately above the rear wing spar. Both wings lower surfaces and the lower fuselage sustained impact damage.
