From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) On October 22, 2006 at approximately 0630 LCL, N22342, a Sikorsky SK-76A++ aircraft crashed while approaching a helideck for landing. The prevailing ceiling and visibility was 600 to 700 feet ceiling and approximately 10 miles visibility with intermittent thunder showers in the vicinity. Upon arriving adjacent to the landing zone, the flight crew setup the landing by entering a left base with final approach to the north. During final approach the aircraft entered the outer edge of a rain shower, the rain became moderate to heavy and windshield wipers were called for by the PIC. Wipers were called for the second time, again with no response. The PIC diverted his attention inside the aircraft to assist in activating the windshield wipers. The aircraft crashed shortly after. A company flight plan was on file and the electronic flight tracking system, the outerlink system, was operational. Both crew members sustained minor injuries.
